Business Associates/Clinical Training Partners
A Business Associate contract is required with any entity (outside the UUHSC workforce) for whom we disclose protected health information to, so that the entity can carry out, assist with the performance of, or perform on behalf of, a function or activity for the UUHSC.Milton Burbidge (801-587-6175) is the Business Associate and Clinical Training Agreement Project Manager.
The following is a list of documents available to assist you. The Business Associate Algorithm helps to define what a Business Associate is. The Business Associate Flow Chart provides an easy to follow way to identify if an entity is a Business Associate. You will receive the Department Letter when you are contact about information sharing.

Clinical Training Partners
List of entities with whom the UUHSC has a Clinical Training Agreement with**(Excel Format).**Properly registered and referred students from these entities may complete their training in our Hospitals and Clinics.
