Alternate Method of Communication

You have the right to request that we communicate with you about your personal health matters in a particular way or at a particular location. For example, you can request that we only contact you at work or at a friend's house.

To request that we use an alternate method of communication:

1.	Complete the Alternate Method of Communication Request Form, but do NOT sign it, yet. Click here for the form.
2.	Have the form notarized or the signature witnessed by a member of University Health Care. Click here to see our requirements for witness or notarization of your signature.
