Request an Amendment to Your Health Information
You have the right to request that we amend your health information if it is incorrect or incomplete.We may deny your request for an amendment if the request does not include a reason to support the request. We may also deny your request for amendment for any of the following reasons: 1) it was not created by the UUHSC, unless you can show that the provider who originally created the health information is no longer available to make the amendment; 2) if the health information is not normally available to the patient, such as psychotherapy notes; 3) or the health information is accurate and complete.
To request an amendment to your health information:
